Chronic Disease, Part II

We hope you enjoyed part 1 of the dual issue about chronic diseases and interventions. We are pleased to present the 2nd part herein. In this issue, we focus on institutional responses to those chronic conditions. We feature exciting material from our colleagues at the Delaware Division of Public Health, the ACCEL clinical and translational research program, University of Delaware, Christiana Care, and the YMCA of Delaware. We need strong institutions and programs to address the breadth and severity of conditions such as diabetes. Using a variety of approaches, including community engagement, applied research, improved care delivery, outcomes research, and prevention methods, the enclosed articles provide a path forward to high-quality interventions. As a reminder, we have developed a new website on chronic diseases, including a rich source of Delaware-specific data.
